BY MAIL:
Please print out the below generic entry form. We have added a space for your e-mail address. Please fill that out, so that we may send you a confirmaiton e-mail that we have received your entry and supporting documents. If you want to pay by credit card, fill out the credit card payment form below and mail back to us no later than April 22nd. NO LATE ENTRIES WILL BE ACCEPTED. IF YOUR ENTRY IS POSTMARKED ON APRIL 22ND, WE WILL NOT BE ABLE TO ACCEPT IT. APRIL 22ND IS THE LAST POSSIBLE DAY FOR US TO ADD ENTRIES TO THE COLLECTORS BOOK AND HAVE IT PRINTED IN TIME FOR THE NATIONALS!!!!!

Mail all entries, judges dinner forms, supporting documents and credit card payment (if using a credit card) to:

BY FAX :
We are sorry, but no entries will be accepted by fax. You can mail or e-mail your entries in. Both require e-mail addresses, so that we can send you a confirmation.

BY E-MAIL:
Send all supporting documents to: USRCNationals@aol.com . Your entry will not be confirmed until you send your payment by mail OR your credit card is processed and approved. Just sending in entries via e-mail, without payment, are not considered complete. If you have a problem printing out these forms, e-mail us and we will send you a pdf of the form immediately.

**NOTE - The below generic entry form also has a section for the Protection Tournament .

**NOTE - All credit card payments MUST fill out this form entirely. We appologize, but 3% merchant account fee must be processed with your credit card payment.
